If you've compared hair transplant clinics, you've met the acronym wall: FUE, DHI, Sapphire, and a dozen trademarked variations. Clinics wield these terms like weapons, each implying the others are obsolete. The truth is calmer: the main modern techniques are closely related, each has cases where it genuinely shines, and the skill of the team performing it matters more than the name on the brochure. The secret clinics rarely lead with: DHI is FUE
Every modern technique starts the same way: individual follicular units are extracted one by one from the donor area at the back and sides of your head, using fine micro-punches. That extraction step IS FUE — follicular unit extraction. DHI ("direct hair implantation") uses the same extraction; what changes is how the grafts go back in. So the real comparison isn't between two different surgeries — it's between two implantation methods on top of the same foundation.
How FUE Sapphire works
Classic FUE has three phases: extraction of the grafts, opening of the implantation channels in the recipient area, and placement of the grafts into those channels. The "Sapphire" part refers to the channel-opening step: instead of steel blades, the channels are opened with sapphire-tipped blades. Sapphire blades are commonly described as producing smoother, more precise channels — which is associated with less tissue trauma, faster healing of the recipient area, and the ability to place channels closer together for a denser look. The channels also fix the angle and direction of each graft, which is where a natural-looking hairline is won or lost.
How DHI works
DHI skips the separate channel-opening phase. Extracted grafts are loaded into a spring-loaded implanter pen (often called a Choi pen), which opens the channel and places the graft in one motion. Typical advantages cited for this: the recipient area often doesn't need to be fully shaved (relevant for women and for small touch-ups), and the pen gives the operator direct control over depth and angle at the moment of placement. Typical trade-offs: sessions are usually slower, large graft counts may need to be split across sessions, and pricing tends to sit higher — the pen work is labor-intensive and depends heavily on the individual operator's skill.

So which one is right for you?
- DHI tends to make sense when the area is small and targeted, when not shaving the recipient area is a priority, or for certain density refinements — provided the operator is genuinely experienced with the pen.
- FUE Sapphire tends to make sense for the most common case we see: noticeable male-pattern hair loss where real coverage is needed — a receding hairline, a thinning crown, or both — treated in a single session.
- For most patients with advanced thinning, the deciding factors are coverage and graft count per session — which is why FUE remains the workhorse of hair restoration worldwide.
And FUT? A quick word on the older method
You may also see FUT (follicular unit transplantation, the "strip" method), where a strip of scalp is removed from the donor area and dissected into grafts. It can move many grafts at once, but it leaves a linear scar across the back of the head and involves a tougher recovery — which is why extraction-based methods like FUE and DHI have largely replaced it as the default, particularly in Istanbul's high-volume hospitals.
